Privacy statement
Please note: this statement is still being worked on. Anything in square brackets will be filled in once the details are known.
Mind-Body-Therapie Valk is responsible for the processing of personal data as set out in this privacy statement. Personal data is processed carefully and in accordance with the requirements of the General Data Protection Regulation (GDPR).
Your interests are at the centre of the support. To be able to support you well, it is necessary for Mind-Body-Therapie Valk to keep a file about you. This is also a legal obligation under the Dutch Medical Treatment Contracts Act (WGBO/Wkkgz). The file contains notes about the state of health and information about any examinations and treatments carried out.
The file also contains information that is necessary for the support and that Mind-Body-Therapie Valk has requested from another care provider, for example your general practitioner, after your explicit written consent.
Mind-Body-Therapie Valk makes every effort to safeguard your privacy. This means, among other things, that:
- personal and medical data is handled with care;
- care is taken to ensure that unauthorised persons have no access to this data.
Paula Valk is the only person with access to the data in the file and has a legal duty of confidentiality (professional secrecy).
The data in the file may be used for the following purposes:
- to inform other care providers, for example when the support is concluded or on referral to another practitioner — this only happens with your explicit written consent, unless the law provides otherwise;
- for anonymised use during peer review;
- a small part of the data is used for the financial administration, so that an invoice can be drawn up.
If your data needs to be used for other reasons, you are informed first and explicitly asked for your written consent. You have the right to inspect your own file at any time and the right to have your data deleted. As required by law, the data in your file is kept for 20 years from your 18th birthday.
Your privacy on the invoice
The invoice you receive states the following details, as required by the Dutch tax authorities:
- your name, address and place of residence
- a short description of the support
- the cost of the session
- the date or dates of the support

This website
This website is deliberately kept simple. When you view a page, your browser makes no connection to any other party: the fonts, the images and the map all sit on the website’s own server. Visitor numbers are not measured and no visitor profile is built up.
Cookies
This website sets no cookies and stores nothing in your browser. That is why you see no cookie notice.
The contact form
Through the contact form you can leave your name, your email address, optionally your phone number, and your message. These details are used only to answer your question or to arrange an appointment.
Your message may say something about your health. That is a special category of personal data. The processing therefore rests on the explicit consent you give with the checkbox below the form (Article 6(1)(a) and Article 9(2)(a) GDPR). You can withdraw that consent at any time.
The form stores nothing on the web server and records no IP address; your message is passed straight to the practice’s mailbox. Whatever belongs in your file is transferred there, after which the email is deleted. Messages that do not lead to an appointment are deleted after three months at the latest.
You do not have to use the form. You are always welcome to call or send an email yourself.
Hosting and server logs
This website and the mailbox are hosted by IONOS SE, Elgendorfer Str. 57, 56410 Montabaur, Germany, in a data centre in Germany. A data processing agreement is in place with IONOS. The data therefore stays within the European Union.
When this website is accessed, IONOS automatically processes data about the access. This includes in particular:
- the previously visited web page (referrer);
- the web page or file requested;
- browser type and browser version;
- the operating system used;
- the type of device used;
- the time of access;
- the IP address in anonymised form.
This processing serves in particular to ensure the security and stability of the website and the technically reliable provision of the service. The legal basis for the processing is Article 6(1)(f) GDPR; the legitimate interest lies in providing this website securely, stably and free of technical faults.
The visitor data collected by IONOS is stored for a period of eight weeks.
Practice software
mijnDiad is used for scheduling appointments, keeping your file and issuing invoices. [ Still to be added: data processing agreement and location of the servers. ] This software is separate from the website; you only encounter it if you are actually receiving support.
Financial administration
[ Still to be added: whether an accountant is engaged and, if so, which data they receive. ]
Map and links to other websites
The map on the contact page is an ordinary image, created from OpenStreetMap data. No map service is loaded and no data leaves the site.
The “Plan route” button is a link to Google Maps. Only when you click it do you leave this website, and Google’s terms and privacy policy then apply. The same goes for the link to the nutrition practice. Mind-Body-Therapie Valk cannot take responsibility for other people’s websites.
Your rights
In respect of your personal data you have the following rights:
- access to the data held about you;
- correction of details that are wrong;
- erasure of your data, in so far as the statutory retention obligation does not stand in the way;
- restriction of processing and objection to processing;
- transfer of your data to yourself or to another party;
- withdrawal of consent given earlier.
You can make a request at paula@mind-body-therapie.info or +31 6 12984035. To prevent data reaching the wrong person, proof of identity may be requested. You will receive an answer within one month.
Complaints about how your data is handled
If you disagree with the way your data is handled, please feel free to raise it with Paula Valk first. If you cannot resolve it together, you have the right to lodge a complaint with the Autoriteit Persoonsgegevens, the Dutch data protection authority.
Changes
This privacy statement may be amended when the website or the way of working changes. The current version is always on this page.
